Recovery doesn’t end when a program does. A relapse prevention plan helps you recognize warning signs early and respond before a slip becomes a setback.

Know your triggers

Triggers are the people, places, feelings, and situations that increase the urge to use. Common ones include stress, certain social settings, and difficult emotions. Naming yours is the first step to managing them.

Build your toolkit

  • Coping skills you can use in the moment — grounding, calling a sponsor, leaving a situation.
  • A support network of people you can reach quickly.
  • Healthy routines — sleep, exercise, and structure reduce vulnerability.
  • An action plan for high-risk moments, written down in advance.

Keep support in place

Aftercare, ongoing counseling, and step-down outpatient care keep you connected. Many people continue with a supportive outpatient program or alumni groups after IOP.
